LE LED bulbs comes with a 3 years standard warranty. You can extend standard cover to a 5 years period by submitting the Warranty Form within 180 days of purchase which will entitle you to:
Unlimited number of replacements for failed bulbs for the duration of your warranty. A brand new replacement of the latest equivalent model if we are unable to offer the model first purchased.

I did not find the warranty form; I'd like to complete that and extend my warranty, as I will be ordering more products for interior and exterior illumination projects. Please tell me where to find this form, or if you can email it to me, that will also work.
